Financing
During your initial consultation, we will provide a personalized treatment plan and an estimate of fees. We will also begin the insurance filing process and provide some clarity about the reimbursements you can expect, if applicable.
Our office accepts multiple forms of payment, and we are always happy to discuss different options that might meet your needs. We accept cash, check, VISA®, Mastercard®, American Express®, Discover®, and CareCredit®. To control billing costs, we ask that all accounts be paid within 60 days of the service date. If your account goes over 60 days, a finance charge of 1.5% per month (18% per year) will be added to the unpaid balance.
Insurance
Our insurance coordinators work with many insurance companies and are committed to providing you with the most accurate estimates possible for potential reimbursements. Please promptly notify our office if there are any changes to your insurance coverage.
Co-Payment: All co-pays are due when you check in at the front desk prior to your appointment.
HMO Patients: If you have a problem that you expect to be covered by medical insurance (such as biopsies, tumors, infections, or abnormalities), you must have a referral from your primary care physician. A referral from a dentist is not adequate for medical insurance coverage.
Medicare Patients: Medicare pays us directly for your care. If Medicare declines to pay for your procedure, you are responsible for additional charges, deductibles, and coinsurance.
Private and Group Insurance: Note that payment is due at the time of your appointment, including any copays or coins. We accept insurance at our Fort Worth location only, as our Colleyville location is for cosmetic procedures only.
